By signing the form below, the party is acknowledging that Muslims are required to be united, and the person or party agrees to work towards unifying the Ummah(Nation) within his/her organization and in conjunction with other organizations that may follow differing schools of thought but who all

1. Witness the Oneness of Allah
2. Witness that Muhammad is Allah's Messenger, the Last Prophet and the   Seal of the Prophets
3.  Pray 5 times daily
4.  Fast During the Month of Ramadan
5.  Pay Zakat
6.  Have made or intend to make Hajj
